Please Note: Payment for cannabis evaluation does not automatically guarantee issuance of a certification card.
No refunds will be provided in case of certification denials. Please review Your State specific cannabis program requirements PRIOR to your appointment.
Offered for patient's with qualifying health conditions under NH and ME state law.

Required for evaluation: State issued ID/Driver's License. Previous documentation confirming or supporting your qualifying medical condition.

ME evaluations: Initial evaluations and re-certifications may be done via telehealth. If qualified, you will receive an electronic copy of your certification card same day. Your hard copy of the card will be mailed to you within 7 days. If evaluated in person, you will be provided a card in hand, same day.

NH evaluations: Initial evaluation MUST be in person. Re-certifications thereafter may be via telehealth.
The state of NH charges a separate application fee of $50 which is not included in the evaluation fee. There is a patient application that needs to be completed in addition to the provider certification. The forms are then mailed (by the patient) to the state of NH for review. If you are approved by the state, a certification card will be issued by mail directly from the state of NH.

Group sessions can be brought to a location that fits best for your event in the Southern Maine/Seacoast New Hampshire region. A $50 mobile fee will be assessed.
