Comprehensive Initial Consult: $199
Includes: Comprehensive exam, initial labs with review ($90 value), basic diet counseling, and prescription sent to pharmacy based on treatment choice if qualifying.
Medication and supplies not included in comprehensive consultation fee.

GLP-1Brand Name Prescription for qualifying patients:

This fee includes monthly tele-health follow ups, and prescription sent to direct to your pharmacy.
​If you have qualifying medical condition (BMI >30, and one comorbidity, or with obstructive sleep apnea) and you have prescription drug coverage:
(please confirm with your insurance company if they cover weygovy/zepbound/ mounjaro)​
We will assist with completion of a prior authorization if required ( for an additional $50.00 fee).​
Semaglutide Brand Name or Tirzepatide Brand Name ​if approved, we will prescribe this medication for you and manage you on this medication​.
Labs are REQUIRED to be done every 3-4 months.

Direct Brand Name GLP-1 Self Pay Program​​​
Monthly Member Management Fee $99.00
​This fee includes monthly tele-health follow ups, and prescriptions sent to program of choice.
You will then pay the pharmaceutical company directly and they will mail medications to you. ​
Same qualifying conditions of BMI >30 and one comorbidity. Good option for if your insurance WILL NOT cover these medications or you do not have insurance.
​Two Options available:​
Semaglutide (Weygovy) Direct Pay Program the cost is $499/ month​
or ​Tirzepatide (Zepbound) through Lilly-Direct
the cost is $349/month for 2.5 mg $499/month for 5 mg, 7.5 mg, 10mg. ​
These medications are provided in 4 single dose vials per month requiring monthly prescriptions, which are included in monthly membership fee.​​
Labs are Required every 3-4 months for safety monitoring. If labs are not completed when due, no prescription will be provided.

Complete Intake Forms Online

Request forms below, or through patient portal by clicking "book now" link.
STEP TWO:
Complete Lab Work

Schedule an office visit for lab draw OR we will email an order form, and go to any LABCORP office to complete.
STEP THREE:
Comprehensive evaluation either ONLINE or IN PERSON (If you opt for lab draw in person, this can be done at same visit)
STEP FOUR: Medications delivered to your home if you qualify for treatment
